To facilitate administrative procedures, please arrive 15 minutes prior to your first appointment with the patient form already printed and filled out:

In order to better assess you pain, you can also fill out the form concerning your pain before coming:

Lesley and Nazyla will answer all your questions about appointments, invoicing and courses. Do not hesitate to contact them by phone or leave a message on the answering machine.

 

If you can't make your appointment, please let us know at least 48 hours in advance. Otherwise, you will be invoiced for your late cancellation.

Finding your way around

The "CENTRE ADMINISTRATIF" is at 17-19, Chemin du Champ-d'Anier.

There is a ramp for wheelchair access or rollators.

Go up the few steps to the mezzanine floor or take the lift.
